Please submit (1) picture of your ID / Passport and  (1) picture of you holding that ID / Passport next to your face like a selfie. Make sure both images are clear and all information is visible. This is a requirement for all deliveries. Thank you for understanding. Please also include your PDF or screenshot of your Medical Recommendation Letter if you have one.
